TMF is a member of the team that supports the Provider Based Learning Systems contract, which includes both Primary Care First (PCF) and the Enhancing Oncology Model (EOM), for primary and specialty care practices that have already developed advanced care capabilities. The purpose of this learning system is to help accelerate outcomes for participants, as well as their model partners, by providing opportunities for sharing and collaborating over issues that are critical to participant success. PCF will be tested over six performance years in up to 23 regions, with two staggered cohorts of participating practices. EOM is a five-year voluntary model, beginning on July 1, 2023. TMF provides practice transformation and payer engagement subject matter expertise, as well as contributes to the management of the PCF Connect networking platform. TMF also assists with the development of the integrated learning system plan, engagement and recruitment strategies, learning events and case studies.
