TMF is a member of the team that supports the Primary Care First (PCF) learning system contract. The purpose of this learning system is to help accelerate outcomes for participants, as well as their model partners, by providing opportunities for sharing and collaborating on issues that are critical to participant success. PCF is designed for primary care practices that have already developed advanced primary care capabilities and are prepared to take on greater financial risk in exchange for reduced administrative burden with the possibility of higher performance-based payments. PCF will be tested over six performance years in up to 26 regions, with two staggered cohorts of participating practices. TMF provides practice transformation and payer engagement subject matter expertise and contributes to the management of the PCF Connect networking platform. TMF also assists with the development of the integrated learning system plan, engagement and recruitment strategies, learning events and case studies.
