For forty years, TMF Health Quality Institute has helped thousands of Texas physicians with advocacy and education.
Throughout all of our work we've concentrated on improving patient care while serving Texas patients and physicians. We've been able to do this because of our strong established relationship with the Texas physician community.
As the CMS Quality Improvement Organization for Texas, TMF is conducting several initiatives to improve patient care.
Our current projects focus on protecting Medicare beneficiaries, promoting patient safety, encouraging preventive health care services and reducing hospital readmissions by improving care transitions.
And TMF has a new program to help physicians help their patients better manage their diabetes.
We also work with Texas physicians through other contracts, such as our ongoing work with the state of Texas in its Vaccines for Children program assisting primary care physicians through education and assessment.
TMF has helped thousands of physicians assess health information technology options. They help doctors select, adopt and implement EHRs. TMF also helps physicians implement care management processes using certified EHRs. CMS also has turned to TMF to raise flu and pneumonia immunization rates, and to help improve care management processes and mammography and colorectal cancer screening rates. TMF also works with physicians to help them get the most out of their EHRs and earn federal stimulus reimbursement. Working with the Texas Regional Extension Centers, TMF provides technical assistance for thousands of primary care physicians. TMF staff also is working on a chronic kidney disease project. They're one of just ten organizations nationwide receiving CMS funding. TMF helps primary care physicians, endocrinologists, nephrologists and other specialists to improve CKD diagnosis and management.
TMF Health Quality Institute is committed to improving quality of health care for all Texans. We believe this is best done through continuing relationships that we have established with the Texas physician community who share the same goal.
